We are collecting real-world examples of Value-Based Health Care (VBHC) implementation across Europe to build a practical library of initiatives and share insights for learning and replication.
We invite you to submit an initiative you are involved in or know well. This may include changes in care delivery, outcome measurement, payment models, or organizational structures. Measurable results are welcome but not required.
VBC initiatives aim to:

- Focus on what matters to patients
- Optimize resource use
